Leslie 122RV - identify and source bolts please!

    My vintage Leslie 122RV has suffered a bolt failure on the horn spindle plate assembly, the bearing plate no longer sits flat, it wobbles like crazy and is unusable.
    I think I have 2 issues:

    1). Some part of the bolt/plate assembly is no longer holding tight - maybe the bolt thread is stripped, or thread on the plate is stripped, or the spacer is no longer making proper contact with the rubber grommet or plate
    2). The head is stripped on the bolt circled in red in one of the photos.

    Can anyone identify what has gone wrong here?
    Also - can anyone help me to source replacement bolts? Or a replacement bearing plate too if necessary?

    The driver is a replacement "Etone", I'm not sure how it is mated to the bearing plate as I've never seen an original V21 assembly.
    I like this driver - it spits and growls and handles much higher power than the V21.
    I'm a big fan of Jon Lord, that's my main tone.

    Lastly, can anyone date this assembly from the various markings, thanks!

    The traditional way this is mounted is the spindle plate sits on a large rubber grommet as shown in the center. Then, the three screws hold the plate firmly against the center grommet. There are metal standoffs on each screw that prevent the screw from totally locking the plate down. The plate should be isolated with rubber at all points. However, it should not rock or tilt.
    Typically, the grommets shrink and get too soft from all the oil that gets on them and age. They should be replaced. It looks like you are missing a second rubber grommet on top of the spindle which resides under the flat metal washer the rotor bearing sits on.
    With new grommets, sitting the plate down and installing the screw standoffs, the top of the standoffs should be nearly flush with the three grommets.
    As for screw replacements any hardware store should be able to match threads etc.

    A big problem with replacing V21's with alternative drivers and re-using the V21 mounting plate is sound leakage. Finding a driver that properly matches the mounting plate is impossible. I recently had to replace such a setup because the rotor sound was being compromised by all the audio leakage from the driver masking the tremolo.
